A truck was leaving a truck stop just outside of Edmonton, Alberta, carrying a hydraulic drilling rig. The truck had a jeep style trailer attached.

As they exited the truck stop, a major component of the trailer carrying jeep failed. The trailer jackknifed around the truck, pulling the entire vehicle to the side of the service road.

When the trailer reached the edge of the pavement it stopped its sideways motion and tipped, causing the trailer together with the chained down hydraulic drilling rig to roll on its side.

Various contractors were called to the scene, to lift the truck and crane, to cut the fouled hoist cables, and to clean up the spilt oil and diesel fuel  from the accident.

The insurance company was requested to pay for the contractors as well as the cleanup.

An initial assessment was performed to evaluate the repair cost. A secondary assessment was performed to evaluate the cost claims of the contractors and the salvage value. Finally, an analysis of the jeep trailer was required to determine what caused the failure.

It was determined that the cause of failure could be attributed to either a design error or negligence in quality control from the original manufacturer of the fifth wheel.
